This Agreement applies to Applicants under eighteen (18) years of age and must be signed by a Parent or Legal Guardian.

PAYMENT RESPONSIBILITY

Parent/Guardian accepts full financial responsibility. All fees are non-refundable.

CONSENT

Parent/Guardian consents to the minor’s participation, photography session, and showcase (if eligible).

GOVERNING LAW

Nevada law governs this Agreement.
